Requirements for Initial Review

  1. Cover Letter addressed to UPLB REB Chair, endorsed by adviser and unit head
  2. Registration and Application Form [UPLB REB FORM 2(B)]
  3. Review Checklist [UPLB REB FORM 2(A)]
  4. Study Protocol Assessment Form [UPLB REB FORM 2(C)] in MS Word format
  5. Study protocol in PDF format
  6. Diagrammatic workflow
  7. Data collection forms
  8. CV of Lead Researcher (LR) and study team members
  9. Informed Consent Assessment Form [UPLB REB FORM 2(D)] in MS Word format
  10. Informed consent form in English
  11. Informed consent form in local language
  12. Basic Research Ethics Training Certificate of Lead Researcher, Co-researcher, and the rest of the study team
  13. Recruitment advertisements (as needed by the study protocol)
  14. Other information or documents for participants (such as diaries, etc.)
  15. For Graduate Students: UPLB GS Thesis/Dissertation Outline Approval Form signed by Adviser and Guidance Committee Members

1. Email one (1) set of properly labelled and ordered soft copies to: [email protected]

Except for UPLB REB Forms 2C and 2D which should be submitted in MS Word format, all other required documents should be submitted in PDF format. The UPLB REB Secretariat performs the initial assessment on the completeness and accuracy of the submission package. You will receive an email notification regarding receipt of documents or screening issues that need to be addressed.

2. Once cleared by the Secretariat, you may submit one (1) set of properly labelled, ordered, and enveloped printed copies addressed to:
